Paxton is tortued by a German man who seems very nervous and reluctant. After several minutes of waffling, the German charges at Paxton with a chainsaw. He cuts through Paxtons hand, but accidentally cuts through the cuff as well. He charges at Paxton again, but this time slips on Paxtons severed fingers and drops the chainsaw on his leg, cutting it clean off. Paxton manages to grab a gun off the table and kill the German. Paxton eventually escapes only to meet an American businessman who is about to torture someone. Paxton makes it to a car outside, but he hears the Japanese girls screams inside. He returns, only to find the American burning her eye. Paxton kills him, and finds the Japanese girls eye dangling out of its socket. He cuts it off. They escape, and along the way find the Natalya, Svetlana, and Alexi talking in the middle of the road. Paxton runs them all over. The two make it to a train station only to find it swarming with cops. The Japanese girl, disfigured, creates a distraction for Paxton by jumping in front of an oncoming train. Paxton jumps on a train only to find the Dutch businessman talking to more backpackers. Paxton follows him into the can at their next stop, and cuts off his fingers. He then shoves his head into the toilet and slits his throat.

Merenda called Edwige Fenech and asked her how she enjoyed working with Roth, and her response was so overwhelmingly enthusiastic that Merenda agreed to do it. Now a successful top dealer, Luc Merenda last was seen on the 1992 television show Edera (1992), starring Susana Bequer, who plays The Translator in the scene with Merenda in Hostel: Part II (2007).

For the scenes in the brothel that Stuart and Todd visit, Eli Roth was keen to film at a genuine brothel in Prague and eventually decided on Big Sister. It was famous for offering customers no charge for sex with the prostitutes, which would normally include fellatio and vaginal sex as well as kissing and licking on the women's bodies (including breasts), as long as the men wore a condom and agreed to live Internet streaming of their activities, which were viewed by paying subscribers. On his solo DVD commentary, Roth suggested Hostel fans should subscribe to the Big Sister website and watch the videos. The brothel closed in 2010 and the website is no longer active, but several videos from the place are easily accessible online for free, including Amateur Czech Babe - Hardline and Czech Out This Babe - Hardline.

The film continues two features from the original Hostel (2005) - all "clients" who have at least one establishing scene before they arrive at the torture factory end up being killed, and the young men and women who deliberately deliver victims to Elite Hunting are themselves murdered. In the former case, Todd is mauled to death by guard dogs after refusing to murder Whitney; after Stuart does kill Whitney, he is disarmed by Beth and presumably bleeds to death when she becomes a client and castrates/defenestrates him (death also befell the Dutch Surgeon, German client, and American Businessman in the original). In the latter case, Axelle is decapitated by Beth in the final scene; Axelle's fellow recruiters were deliberately run over by either Paxton or the thugs pursuing him (before they were killed by the Bubble Gum Gang) in the final part of the original.

Heather Matarazzo had been sober for about a year when she arrived in Prague and attended a "get to know you" party prior to shooting. She claimed she was constantly offered pills by the producers to help her beat the jet lag and "get some sleep", but she repeatedly turned them down. She also alleged that when Bijou Phillips found out she was sober, she pushed her against a wall and choked her for 15 to 20 seconds and said, "I'm going to make you relapse on this film." Matarazzo said she was too embarrassed to tell the director or producers about the incident.
