Adam Rifkin has said that he originally set the film in New York City and Philadelphia. After a scouting trip and the team reportedly "falling in love" with the state, Rifkin re-wrote the film set in Nashville and Knoxville. Both Rifkin and Whitner have since remarked that Tennessee truly became another character in the film.

The movie changed its name from Dog Years to The Last Movie Star in December 2017. A24 and DirecTV obtained the North American distribution rights to "The Last Movie Star" in June, and that's where the name change came from, according to Gordon Whitener, one of the film's producers. "When they first approached us about buying the film and the North American rights, they said 'Look we may want to think about a name change,' and so we looked at a couple of different ones, and we kind of all settled on 'The Last Movie Star,' " he said. Whitener said he likes the new name because it fits what he felt while working on "The Last Movie Star." "When people would ask me what it was like to work with Burt Reynolds ... my comment would be 'it's like working with one of the last real movie stars ... from the old days' so we're pretty excited about it. I think ... everybody feels that it's a better fit for taking something out and marketing it and release, and so that was really what drove it."

When Vic is asked about why he went from Marlon Brando style character-studies to 'actioney' pictures, he says, "I turned down Serpico for Johnny Speedway." Al Pacino was in Serpico in 1973, the same year Burt was in what many (including himself) consider his last great character-driven film with serious acting, Deliverance, although it does involve a lot of action.
