The hockey game scene was filmed at the first (and at the time only) Ice facility in Columbia, South Carolina. The hockey players seen in the film are (or were) members of hockey programs throughout the area. Scenes in which you cannot see actor Stuart Lafferty's face but his character, Brendan Hume, is present, local hockey player Sam Waller was used as a body double; Waller was uncredited. Some of the extras that were seen during hockey action sequence were the same that can be seen in an earlier scene (on the unrated version on DVD), when Kelly Preston (Helen Hume) and Jordan Garrett (Lucas Hume) are discussing an in class conflict, the camera shows Brendan Hume hanging out at school with his teammates. Once again a few of the same hockey players can been seen once again during the scene of Brendan's Funeral.

Leigh Whannell has been involved in nearly every one of James Wan's films. In this movie he plays Spink, one of the members of Billy Darley's gang. Leigh had also co-wrote and starred in Saw (2004) and co-wrote Dead Silence (2007).

The courtroom, in which the pre-trial hearing takes place, is not actually a trial court. It is the South Carolina Court of Appeals. No pre-trial hearing or trials take place there.

During production of the hockey scene, actor Stuart Lafferty was nearly injured when he was standing on the ice leaning against the goal as the extras, who were actual hockey players, warmed up. Lafferty was initially called off the ice by the parent of an extra who was concerned for Lafferty's safety. The parent was confronted by a member of the production crew who stated "Sir you are not permitted to talk to the talent" to which the parent responded with "alright, well your star is leaning up against the net that these players are shooting pucks at...I do not think you want your star to have to go to the hospital with an apparent head wound from a rogue puck hitting him in the face", the production member then quickly called for Lafferty to leave the ice.

Originally, the film was to take place in Boston, Massachusetts, before Director James Wan decided to have the film set in Columbia, South Carolina. The novel, completely different from the movie, took place in Chicago, Illinois.
