"Old Nick," the name of the antagonist, is another name for the Devil in Christianity, dating back to the mid-17th century, most likely associated with Niccolò Machiavelli. Emma Donoghue, the author of both the source novel and the movie's screenplay, has confirmed in several interviews that this was her purpose for choosing this nickname for Ma and Jack's captor. For example, in 2010 (shortly after the book's publication), Donoghue told the public radio interviewer Diane Rehm, "their captor...is known only by the nickname Old Nick, because he's very much a devil figure," and she told the faith-based website Patheos, "because it's a mother raising a child who is potentially heroic, I saw it not only as being like Mary and Jesus with Old Nick as the Devil - in Britain, 'Old Nick' is a slang phrase for the Devil - so yeah, I saw the Mary and Jesus elements as pretty obvious, but also kind of Adam and Eve elements. To me it was important for Jack to be male. I liked the idea that they act as this kind of microcosm of human civilization, this male and female. And also as a contrast to Old Nick; he's such a devilish figure, I thought having a really good male figure, in the form of Jack, kept that balance."

In some scenes in Joy's teenage bedroom, a poster from the famous band Radiohead's OK Computer album era is shown. The poster reads the lyrics of a track from OK Computer, "Fitter Happier", in which a robotic voice speaks about "modern human" in an ironic and criticizing way, and ends with the lines "fitter, happier, and more productive, a pig, in a cage, on antibiotics". There could be thousands of possible number combinations, and she doesn't know how often her captor changes the code and/or if a certain number of wrong guesses would trigger an alarm. There are similarities between this film and the Napoleonic-era novel The Count of Monte Cristo by Alexander Dumas. In the book Edmond Dantès is the victim of a plot by evil men and, being framed as a presumed traitor, he is imprisoned in a solitary cell in a remote fortress. After many years an elderly prisoner breaks through the wall of his cell, having spent most of a decade trying to tunnel his way to freedom. Later, realizing that he will not live to see freedom, the old man tells Dantès of a vast treasure and how to find it.When the old man dies, he is sewn into a sack to be buried at sea. Dantès finds him in this condition, hides his body in the tunnel, and trades places with him, sewing himself into the sack. Once the preparations are ready, the jailers return and he is thrown into the ocean, then cuts himself free and escapes. After being found by pirates, he finds the treasure and assumes leadership. He then presents himself as a wealthy count and methodically takes out his revenge on those who had imprisoned him.Joy / Ma tells Jack the story of the Count of Monte Cristo. When she rolls him into a carpet and shows him how to play dead, and then escape, she is taking the idea directly from Dumas' story.

While director Lenny Abrahamson was looking for an actress to play "Ma," an associate suggested that he see Brie Larson in Short Term 12 (2013). He was instantly impressed with Larson's ability to inhabit her character so completely and unpretentiously. This was precisely what he sought for his film.
