Like the film Seventh Son (2014), this movies also uses the real Lancashire region of Pendle (and its eponymous hill) for its supernatural connection. The real-life Pendle witches were tried and convicted of the murder of ten people by witchcraft in 1612 and, along with the Samlesbury witches and others, have come to be known the Lancashire witch trials. Of the eleven who went to trial, nine women and two men, nine were found guilty and executed by hanging; one was found not guilty.

David Harbour had stated that Hellboy has a "very special relationship" with Nimue and that the film expands her role from the comics. Milla Jovovich described the character as being "literally the queen of the underworld", serving as the mother of the monsters and described Nimue's plan as "beautiful" and "relevant" to today's political climate in trying to bring people together.
